7. Criminal record check

Fusion carries out a Ministry of Justice criminal record check for all new staff. Download the Request for criminal conviction history form and complete pages 2–4. The Ministry requires the original signed form, so print, sign and hand it (with a copy of your ID) to your supervisor or HR. You can upload a photo of it below so HR can check it before posting.
